Reason for rejection field

    When approving and especially when rejecting a request or service option, there should be a field available to state the reason for the rejection/approval.

    This field should be available in the events/rules/actions so it can be included in emails sent from Service Catalog.

    This field should also be included in the audit tab so it can be reviewed there and reported upon who rejected what when and why.
