OOTB catalog offering to "report an issue" in SD does not pass contact info

    I have clients who would like the default out-of-the-box offering to "report an issue",

    which creates a Service Desk ticket,

    to also pass phone number and email address.

    Currently it does not pass this information to Service Desk even though it can be entered on the catalog side.
