SQL:  Maps DB field

Unfortunately we have not had the time or resources to maintain a document that shows us what fields we have added or modified over the years and are looking for a query that we could run.

Does anyone have a SQL query that they can share that would show the mapping between the property label and the database table and database column (attribute ID)? Even the data type and lookup ID would be great. Is there a way to know if the field was OOTB vs. a field that we created. Also, if the field is active or not. We don't know what information we may want to know that isn't included yet - as they say you don't know what you don't know...


Also, would you be able to share a query that would provide us details of data in the look-ups, such as look-up name, ID, source, look values and ID. For instance, users see "high/medium/low" but the db captured numbers like "3/2/1" or "1/2/3" it would be very helpful to know that, too.
