Process Error Message BPM-0519

Good morning - I am hoping that someone out here has seen this and knows what it means and how to correct it. . .

BPM-0519: Internal Process Engine Error. Contact your site administrator (Object with name '' refers to object '' bound to the instance '' is not found). 

I have this happening intermittently on our change request process.  When I look at the error in the process it also says that there is no ‘assignee’ to send it to.  When I look at the Change Request, the approver field clearly has a name from the associated lookup in there.  The ‘retry’ does not work in these instances.

Any thoughts on how to prevent or why it may not be seeing that the field is filled in?
