Clarity R12 Upgrade Issue

We have upgrade from Clarity 8.1 to Clarity R12 SP06. The issue we are facing is the running instances of a process of 8.1 Version are now failing whenever a custom script is encountered with error message "Cannot read custom action context from the database"

But the new instance on R12 of the same process is running fine.

Can anyone please let us know if you have faced the similar kind of issue.
