Duplicate Task ID Error

Hello everyone. Has anyone encountered a "Duplicate Task ID error [] Cannot Save to Clarity" when saving back to Clarity? We are seeing this more and more and dont know what triggers it, and how to fix it. Any help is greatly appreciated!  Thank you,  Paul
