Use of escalations within a process

Anybody out there using escalations in a workflow process?      We have started adding escalations to approval steps within a process for things like a Change Request approval that sits unresponded too for more than 48 hours.     What we are finding though is that the escalation is triggered even when the Action Item for the Approval is responded too before the 48 hour time limit is up.     Said another way.   If the Action Item is marked approved 6 hours after the initial request was made, at 48 hours the escalation notice still triggers a follow up Action item.  Anybody using escalations and able to configure them so they only trigger when they are supposed to?  Thanks in advance,Steven
