Is CA Siteminder version 12.5 compatible with CA Clarity PPM 12.01? Is there a documentation available for it?
Clarity was something everyone on SM ran from when i was with CA.
only thing i remember is that most often people would have issues as the application server wouldnt accept headers it did not set.
I am facing similar problems integrating SM with Clarity. Any leads on that?

PPM r12 is no longer supported.
You might want to open a similar question into the Clarity PPM community if this document is not enough to perform the integration.

In my 10 years here in Support at CA Technologies, I've never seen anyone supporting ANY CA product to 'run-away' from anything. If someone feels otherwise, then I would strongly encourage that they contact CA Support Management to discuss their concerns about how they feel their case is being handled.
As for the original question...
The integration between CA SSO and CA PPM should be to protect the CA PPM web based management utility with CA SSO. CA SSO doesn't publish every possible web based resource which can be protected by CA SSO. This is typically dictated by the type of resource (e.g. web server or web application), and the OS it is running on.
The key would be whether the version of CA SSO is currently supported on the OS and version of the web server/web application being protected. If the OEM vendor no longer supports the web server or web application, then CA Support will not support that integration either. This does not necessarily mean it will not work. It simply means we are limited in our course of action to support them. For example, we will not be able to have Dev Fixes developed by CA Sustaining Engineering or get P1 status' and follow-the-sun support.
I just looked through the document and with 12.1 only CA SiteMinder 6.0: any maintenance release is supported. Also CA PPM 12.1 is end of life version.
