We are upgrading to CA SDM 14.1 and noticed on the Issue ticket type when we go to Activities > Update Status, Callback, Research, Log Comment, or Solution that the form doesn't fully load. We rolled back all modified htmpl files and have no SPEL code customization. You can notice that the menu links aren't even loading the css correctly either. In fact, after we setup our PROD environment and applied the patches, we noticed the problem is there too out of the box. The screen shot below is our test environment, but again, we have the same issue in PROD which is un-modified. Has anyone experienced this? It is only with this one ticket type. I believe these use a couple different htmpl forms, but with the css not even loading right it makes me believe the issue is deeper in the code than that.
