custom field added but not showing

Hi Team

 

I have added one new textbox on incident form name as Guest User Name.First i have created  column in cr table then add the attribute in the textbox. all things went successfully.

 

but after creating ticket, when open that ticket , that field information is not showing. please help. what is left to do..????

please urgent help
