how to create report on the form level of CA Service Catalog R12.9. Example I have business unit named "IT" under that I have defined services and associated forms to that. Now I want to get the web intelligence report for the attributes defined on the form

I need to know the table name under mdb which store all the forms data information which user input from CA Service Catalog web interface
