USS_MDB not crfeated during installation of Unified Self service

Hi All,

I encountered this problem while installing USS on a existing SDM 14.1 installation. After the installation, i launched the USS portal and got a blank error page. On checking the database server, i realised that no extra databases were created and the only database is the MDB. Any ideas what can cause this? The sapce on the db server is ok and the db server is located on a remote server.

I also have EEM already installed but not yet integrated.

Also can someone help with the list of extra databases to be installed as part of the USS installation
