"Attempt to add/ update record failed" error is coming when some of the users are trying to convert ideas to projects. Can any one help on this?
It is difficult to tell if this is a rights issue but normally we would have expected to see something that would allude to that in the error. It would likely be advantageous to see the corresponding app-ca.log for the time frame the error occurred. The log may provide more detail regarding the error.
Can you attach the log and let us know the time & date of the occurrence? If you do not know when it was please try to recreate the issue and attach the app-ca.log.

What do you see in the app logs ?
Please check if there is any process which is locking any of the mandatory attributes of that Idea while you try to convert it to a Project.
One more thing that you can check, as that might help:
While converting the Idea to a project, are you using some other project as a template?
If yes, then check if any of the Team records for that Project is having PRAVAILSTART or PRAVAILFINISH columns filled up in the database.
You will get these information in the PRTEAM table if you search by the PRPROJECTID.
Any thing related to rights? Like users did not have "Basic group for user access".
The issue disappeared after proper access rights were given to user over idea and project.
