Does anyone know how the "throughput report" determine what stories and defects to include in the counts and how it determines the dates to include them under? Thanks.

Short answer- it's based on Accepted Date. A work item doesn't count toward throughput until it is Accepted and it gets reported on based on the Accepted Date. Also, haven't looked recently at Throughput reports but some reports won't show items until after the end of the month in which they're accepted. Hope that helps.
