Who creates the team allocation, is it PM or RM?

Both options are possible. 

typically the following works for most organisations:

1. PM enters allocation against role
2. RM replaces role with a named resource (allocation adjusted if needed)

I'll caveat that it depends on the maturity of the organisation and parties involved, get in touch if you want to discuss further
