CA Service Management

Has anyone ever seen a blank page after tying to copy an old form? We're using Catalog 14.1. It seems to load the form name and then everything below it is just blank.

Please let me know the following, to be able to further check ont his one?
- Did you already apply the SC R14.1.04(CP4) patch?
- Does this occur while you 'paste' the copy of the form?
- Does this happen after you pasted the form and try to open the copied form?
  (Although the 'paste' should/does open the copy implicitly).
