Has anyone integrated CA SSO with Microsoft or Google Authenticator applications for multi-factor or step-up authentication?
I am curious to know if anyone has done this, and if so, what mechanism was used to implement it.

Yes. I have not see many customer implementations, but a partner of ours (IDF Connect) sells a google authenticator integration to SSO. SSO/MobileKey | Two-Factor Authetication | IDF Connect
