What is the complete error stack in the startup sequence? IM should upgrade on first startup.  Clearly it can connect, as it knows it needs to upgrade the schema, but it must not be able to make the required changes.

The server log should make clear why this is not happening. The mostly likely scenario is that the user IM is using to connect to the task persistence database does not have the rights to modify the schema. The simplest solution is to have the DBA temporarily grant this user DBA priviledges and let the upgrade complete and then revert back to the original runtime rights.
