Problem with ticket windows after 14.1 - 17.1 Upgrade

I recently upgrade my 14.1CUM2 to 17.1.01 SDM and now everytime i click "edit" on a ticket the window seems like this:



All fields are gone when you click edit.. Anyone has seen something like this?
