Best way to track QA/UAT defects

I am wondering what is the best practice to track QA/UAT defects with in the clarity tool. Some of the features we are looking at are:

1. Ability to create defects, manage defects(prioritize defects, capture conversations between IT and Business Teams, uploading documents with screenshots etc)
2. Ability to tie requirements, defects, IT projects in order to generate some meaningful metrics such as which requirements generated maximum defects(in some cases, high priority tickets) etc.

I understand that all the features may not be available in Clarity, just wondering what is supported out-of-the-box.
