Unified Self Service - Configure Data Sources CA Service Desk Manager 14.1

Hi,

 

I have 14.1 and Unified self service both installed and up and running independently.  I login to the USS interface as an administrator and go to Settings>Data Sources>CA Service Desk Manager.  I fill in all the details, load up the certificate and when I press save or test connection, i'm getting a message saying Required Fields are missing.  I'm not seeing what it's not liking.  Any ideas would be greatly appreciated.
